Medical Forms
Listed below are the required medical forms. Optional forms are available for athletics and special conditions.
Illinois State Eye and Health Forms are due by October 15 for continued attendance, and dental forms are are due by May 15.
Emergency Medical Information Form for Liberty CUSD2 (required - complete and submit by the first day of school)
Illinois Certificate of Child Health Examination (required for Grades PK, K, 6, 9, and MCV4 vaccine for grade 12)
Illinois Eye Examination (required for Grade K and all New Students)
Illinois Proof of School Dental Examination (required for Grades K, 2, 6, 9)
